What to Bring to Your Appointment
Arriving at your appointment with all the required documents makes for a faster, better, and more thorough assessment. If you are not sure you need to bring a specific document/paperwork, bring it. It’s better to have too much information rather than not enough.
Things to Remember When Coming in for an Assessment
- Be prepared to stay approximately 1 hour for the entire assessment process. Do not bring children with you.
- Arrive 10 – 15 minutes before appointment time to complete the initial paperwork.
- Bring all required paperwork – DWI citation, BAC reading (breathalyzer results), and your complete DMV driving history (Motor Vehicle Record) – www.ncdot.org/dmv/records.

Bring the applicable assessment fees:
- Substance Abuse: $150
- DWI: $100 for each outstanding DWI
