Arriving at your appointment with all the required documents makes for a faster, better, and more thorough assessment. If you are not sure you need to bring a specific document/paperwork, bring it. It’s better to have too much information rather than not enough.
Things to Remember When Coming in for an Assessment
- Be prepared to stay approx 1 hour for the entire assessment process. Do not bring children with you.
- Arrive 10 – 15 minutes before appointment time to complete the initial paperwork.
- Bring all required paperwork – DWI ticket, BAC at time of arrest (breathalyzer results), and complete DMV driving history (Motor Vehicle Record).
Call us within 24 hours to let us know if you are unable to keep your appointment.
Bring the applicable assessment fees:
- Substance Abuse: $100
- DWI: $100 for each outstanding DWI
- Out of State DWI: $250
